Go to your subscription center and look for Productivity Tools for AutoCAD® Civil 3D® 2013.

We can't see the Productivity Tools for AutoCAD® Civil 3D® 2013 in the subscription center for downloading.

It appears theat you have a subscription for Revit &/or MEP, not Civil3D. AFAIK, this extension is ONLY for Civil3D.
