I have C3D 2011 and I can't set it to always open a DWG file with C3D as autacad.
Initially I am able to set to open with C3D as autocad but every time I use C3D this configuration is lost.
What should I do?

Yes. When you open a drawing with C3D it sets the registry value to that.
To get around this I always drag the file I want to open from Explorer to the appropriate Icon on my desktop or taskbar. You can also open the program you want and then open the drawing.
You could backup that portion of the registry and restore it each time reboot your computer. But that seems like a lot of trouble.

No, because both "Civil 3D" and "Civil 3D as Autocad" run the same executable. The only difference is the Profile that gets loaded. When you load a "Civil 3D as Autocad" profile, a lot of the C3D-specific modules are not loaded, but otherwise, you are running the same basic executable file.
Unless you start Civil 3D with a shortcut that force-loads a specific Profile, C3D will start up using the Profile you used last.
Now I understood! I din't realized that it was the same executable.
So, how can I do to this shortcut (that force-loads a specific profile)?

If you go to Start > Programs > Autodesk > AutoCAD Civil Civil 3D (your Year) there should be a choice for AutoCAD Civil 3D (year) as AutoCAD. Copy that on to your Desktop and drag a drawing from explorer to there. You can also copy it onto The Quick Launch Toolbar on your Task Bar.
If you don't have this you can create one by altering another AutoCAD icon to contain something like this:
"C:\[program location]AutoCAD Civil 3D (year)\acad.exe" /p "AutoCAD"
The /p "AutoCAD" can be replaced with any profile you want. The "AutoCAD" is the stock one to start Civil 3D as AutoCAD.
