This is what I'm trying to achieve, except with a Civil3D label.
I want to add the circle with the typical detail callout to an alignment label. I can format the text just fine, but can't figure out any way to add a symbol to a label. For example, I was thinking I would create a "water meter" style with the correct number for water meters, so there would be a block or something attached to the label.
Any suggestions on how to do this?

I made this one with two text components and a line component. It doesn't appear as though a block is necessary.
I did notice however the block component is also available for this type of label.
I believe you can add blocks to a style in the Label Style composer. See attached.

I also tried adding a block. It worked, but I had some problems getting it to rotate to my viewport. It seems like using a text object gives me a little more control.
Make a block of the circle and stick with the insert point where the leader will attach. In the lable composer add a block, select the one you made adjust your fields to where they belong
the circle is one of the border shapes you can put around a text component. just set border to true and select circle instead of rectangle.
