Toolbar icons are question marks

I have been using AutoCAD for a long time and this problem only started in 2014 and has continued into 2015.

I edit and save over icons for custom ribbon panels and toolbars. I assign them to the button and when I hit apply and close the change isn't applied. When I go back to the CUI it is back to the old one but I don't know where that old one is because if I try to reassign it to a bmp file with the same name it is the new one and then goes back.

I have made sure that only the good icons are in my icons folder and that the path in Options/Files is assigned correctly. The palette and toolbars are saved in my workspace but it is not helping.

I went to the icons folder in windows explorer and checked them and they are all the new icons.

Can anyone help me out?

Are you selecting the icon and it's path in the CUI? Or just typing in the name?
Why use "the same name"? Why not make a new name or edit/modify the image file outside of AutoCAD using any raster editor out there?

Renaming the icons worked.
