Layerpalette display bug on startup

I am having an issue, upon startup.

1. Load/Unload Customizations opens every time I startup Autocad. Please tell me how I can disable this. I have searched but have not found an answer.

2. After closing the Load/Unload Customizations window, about 50% of the time I get the graphics issue shown in the attached image, where part of the drawing is repeated in some sort of additional model space. It is not usable space, and I am not able to open the Layer Palette.
