Some text and leaders not showing up from ACAD 2005 to 2011

I am working in vanilla AutoCAD 2011, when opening some drawings that were created in AutoCAD 2005 (2004 file format), some of the text and leaders do not show up. Not all the drawings in the set and not all the text or leaders are dissappering.
I'm not sure if the drawings were created with AutoCAD 2005 or 2006 ADT, but seem to be vanilla AutoCAD.
If you open the drawings up in 2005, the text and leaders are still there.
Tried recovering in both 2005 and 2011, tried saving down to 2000, nothing seems to help.
Any thoughts or suggestions?

Here is a test file to look at.
(disclaimer: I didn't create this file, just working with it)
Thanks

See attached: you missing items did not have the 1/8" annotative scale attached to them in modelspace.

Thank you!!
I could have sworn I checked all of that.
I don't use annotative text, so there are some things I don't know to even look for.
(cheers)
