I am working on an old (OLD) version of Auto CAD (no integrated converter). What is the best way outside of Auto CAD to Convert a PDF file to a DWG?

We use PDF-to-DWG Converter.
http://www.autodwg.com/pdf-to-dwg-converter/
It works for us most of the time.
It is supposed to work with scanned-to-PDF images, but we have found it does not.

I am working on auto CAD 2000, with hydroCAD. I have seen a lot of PDF converters out there created by third parties, what has been your experience with autodwg? Does it have any limitations to it's abilities to convert?

Unless it has changed recently, AutoDWG will not convert PDF files that consist of raster images to line drawings. Which is to say, it won't convert most PDF files, other than to produce a dwg file with an embedded raster image.
I have had good luck with Print2Cad from www.backtocad.com. And it *does* work with scanned pdf files (which are raster images).
When I was a little bitty boy, my grandmother bought me a cute little toy. Silver bells hangin' on a string... she told me it was my ding-a-ling-a-ling!

We use a program called pdf2cad and it works really well. First you save a copy of your PDF, then convert your pdf. Thru Autocad then open converted pdf as a dxf in and save to .dwg. You will have to scale back up to 1:1 as it loses it's scale during the conversion process. We use it everyday and save a ton of hrs.
As mentioned above, it will not convert a scanned imaged tagged as a pdf.

This program worked great and its open source. I looked through a stack of the other ones and thought I'd give this a try. It worked great for me so you might want to give it a try. I've been posting it on a bunch of the forum posts about converters because it worked great for me.
Only thing is you have to cleanup a little bit and redo the scaling. Worth a try first though if you're looking through some of these.

AutoCAD 2012 has the capability to insert the PDF as reference. We use Adobe Acrobat and Aide PDF to DWG Converter. Open the PDF file in Adobe Acrobat, you can copy the graphic objects and paste into AutoCAD. Aide PDF to DWG Converter supposed to work with scanned PDF.

Open your Internet browser, download and install the PDFIn PDF to DWG Converter (link provider in "Resources" section). Launch the downloaded software, then click "Add File." Choose the PDF file you want to convert, then select a location to save the file to. Click "Convert" and the PDF file automatically converts over to the DWG file format.
