Filename automatica lly changes on it's own

On one of our host laptops, when the user opens multiple files in separate subdirectories (via the Acad pulldown menu) the file name for the new drawing automatically changes to drawing1.dwg, instead of by the name of the file that he opened.
One of the errors we notice when he has had this problem is that the new file he opened to seems to have lost it's ability to locate an xref.
ie. User has floorA.dwg open and opens floorB.dwg open at the same time. When he switches from floorA.dwg to floorB.dwg, the floorB.dwg automatically assigns itself the "drawing1.dwg" filename.
Any thoughts?

Matt,
As the scenario seems to take place sporadically, I'm not certain. We will type that in the next time it happens and let you know. What we do know, is that when it has happened it shows as the drawing1.dwg filename on the screen and when he goes to saveas it does appear to want to saveas drawing1.dwg instead of the original filename.

If you find out what are the steps to reproduce please post them here.
When you try to save the file, will it save as drawing1.dwg as well?

We have not had this scenario happen in awhile. However, I have seen older posts regarding the read only file access not working correctly. For instance I am in a current drawing session, go to save, then get notified that the original file I was previously is now all of the sudden read only. After checking with others in my office, I come to discover that yes someone else opened the file and was not notified that I was already in the file thus allowing them to proceed with access to the file as if though nobody else were already in it.
Unfortunately the IT dept not much help... but I believe we are running Windows Server 2008 as our NOS & Windows 7 on our desktops/laptops with Acad 2012. Any ideas?

Which type of data server you have?
Is SMB V2 active? Just a thought...
