AutoCAD 2010/2011/2012 DWG Format

I have a drawing where I have placed all of my natural gas piping.  I have all of the annotative blocks set to 3/32"=1'-0" and 1/32"=1'-0" scales.  They are both correctly displayed in this drawing. 

When I xref this drawing into another about half of the 1/32" blocks show up correctly.  The other half are shifted to the right off of the plan.  All of the 3/32" blocks show up correctly.

Any ideas?

The half that messed up probably have their BASE not at 0,0,0

I created them all the same way.  How can I tell if the Base is incorrect?  They are all correct on the xref .dwg file.

I closed the xref file and reopened it to find the blocks that were incorrect in the other file were now incorrect on the xref.  They were not just shifted they are mirrored.  They appear to all be mirrored about the same line but, how do I mirror them back without mirroring the other scale?

Base Point of a block is not the same as Base.

Did you make these blocks when the UCS rotated?

I do everything in 2-D.  I have not intentionally adjusted the UCS.  I have attached the file.

Go check both drawings to see if they're using same UNITS, then type in Base, are they both 0,0,0 ????

The error is on the one drawing "Mechanical" that I posted which is the file that is xrefed into another "Plumbing".  In the Mechanical file after closing and re-opening the file the blocks were messed up.  Most of the blocks that are drawn at 1/32" scale are mirrored and shifted to the left.  The 3/32" scale view shows them in the correct place.  Can I mirror them back into place without mirroring the 3/32" scale view?

Annotation scaled blocks are supposed to be able to have different locations at different scales.  It is a design feature.  There is a danger however of inadvertently moving the blocks in one scale without meaning to.  It probably happened to you.  Turn on the scale where they are displayed in the correct positions.

!!Warning!! Peform the following at your own risk.

Select all the objects in the view you like and on the annotate tab of the ribbon, choose sync scale positions in the annotation scale panel.  The command is annoreset.
