ucs icon changes when zooming in

for some reason my ucs icon changes from the regular model space icon to a strange flattened icon with only the x axis indicated when i zoom in. when i zoom out it returns back to normal.

I'm having exactly the same problem...I'm using 2012 Acad - with SP2 installed. what is the issue here?...can't figure this out

Thanks, I tried Regen and nothing. However, moving the details closer to 0,0 resolved the issue. But I may not be able to do that every time without great impacts. Not sure if there could have been another way to resolve the issue without moving the objects. I did try setting the drawing limits to include the details, but didn't help... Kind of a weird problem...in addition to it some osnaps would not work.
